What Exactly Is Shockwave Therapy?

Shockwave therapy uses focused or radial acoustic (sound) pressure waves delivered through a handheld device placed on the skin. These waves travel into the targeted tissue. Unlike high-energy versions once used mainly to break up kidney stones, the lower-to-medium energy levels used for muscle, tendon, and joint problems are designed to stimulate healing rather than destroy tissue.

Sessions are typically short (a few minutes per area), non-invasive, and done in a clinic setting. Most people feel a tapping or pulsing sensation that can be uncomfortable but is usually tolerable. No surgery, no needles, and most patients return to normal activities the same day.

How Does It Work?

The main way shockwave helps is through a process called mechanotransduction. This is when the mechanical energy of the waves is converted by the receptors on your cells into helpful biological signals. Research shows this can lead to several beneficial effects:

- Pain relief — It can reduce certain pain-related chemicals in the tissue and calm overactive nerve signals.

- Improved blood flow — It encourages the formation of new small blood vessels (neovascularization), bringing more oxygen and nutrients to areas that often have poor circulation, such as chronic tendon problems.

Because pain and tissue quality improve, people often regain range of motion and function more easily when shockwave is combined with appropriate exercise and rehabilitation. What Should You Expect?

A typical course involves 3–5 sessions spaced about one week apart, though exact plans vary. Improvement is gradual. Some people notice changes within a couple of weeks; others see the biggest gains several weeks after finishing treatment. Shockwave works best as part of a broader plan that includes progressive loading, mobility work, and addressing contributing factors (such as training load or movement patterns).

Side effects are generally mild and temporary—temporary soreness, redness, or possibly mild swelling in the treated area are the most common. Serious complications are rare when the therapy is applied by trained clinicians using appropriate energy levels.

Is It Right for You?

Shockwave is not a magic fix, and it is not appropriate for every situation (for example, certain infections, active cancer in the treatment area, or pregnancy near the treatment site are contraindications).
