Elevate PT is born out of a love for the value and effectiveness of conservative care in healing and correcting neuromuscular and musculoskeletal issues. I have found that when done with excellence, Physical Therapy can be a powerful tool to overcome most pain and dysfunction that arises with physical activity.
Unfortunately, our modern healthcare system has gotten to a point where profits come before patient needs. Insurance companies limit payments for treatment times and interventions in order to combat the outrageous billing of huge health systems. Caught in the middle of this mess are hurting people who are trying to get better and burned out therapists who are trying to be better. We are tired of sloppy, rushed, and generalized care and we refuse to let insurance dictate how we treat our patients.

Kurtis McIntire, DPT, CSMT, Cert DN
I’ve always believed that great physical therapy doesn’t just treat your injury—it treats you. At Elevate PT, I focus on the whole person, from muscles to mindset. Understanding the complexity and intricacy of pain is key to recovery, and by addressing how your nervous system and muscles work together, we can often get you pain-free faster than you’d expect.
I earned my Doctor of Physical Therapy from Andrews University, with a special interest in pain science that has shaped my approach to patient care. Continuing education is a big part of how I stay at the forefront of the field, and I regularly follow industry leaders like Greg Lehman, Lorimer Mosley, Peter O'Sullivan, and others. My most impactful certification has been the Certified Spinal Manual Therapist (CSMT), which blends pain science with hands-on techniques that I use every day in my practice.
As for me, I’m a weekend warrior who’s never met a sport I didn’t like. Outside the clinic, you’ll usually find me on the sand volleyball court or hiking with my wife, Hana, and our two dogs, Kona and Bear. If you’re an animal lover, the pups would love to say hi when you visit the clinic!
At Elevate, we believe that the best outcomes come from a real connection between therapist and patient. That’s why I’m committed to providing one-on-one, 60-minute sessions every time—because when it comes to your health, you deserve more than just a quick fix.
Frequently Asked Questions
Do I need a referral for Physical Therapy?
In the state of New Jersey you can see a licensed Physical Therapist without a referral from a physician. We send our plan of care to your primary care physician to keep everyone in the loop. Also, because we are not hindered by insurance requirements, there are no obstacles to getting started immediately.
Do you accept my insurance?
The short answer is no, we are out of network with all insurances due to limitations they put on treatment. Some insurances offer reimbursement for services from out of network providers. All services are eligible for HSA and FSA funds. If you have any other specific questions about that, please call or text us or sign up for a free consultation.
How much does it cost?
Initial evaluation cost is $175. This includes in-depth evaluation and establishing a plan for home program. Follow up visits depend on the plan of care we establish together because we offer discounted packages. You will never have any surprise costs or bills arriving months after treatment.
I have never been to PT before, what should I expect?
Our first visit will be focused on understanding you and your concerns. This includes gathering your health journey history and assessing current or past issues using detailed tests and measures to find both the painful structures along with the root cause of the issue. We also initiate treatment on Day 1, including steps to resolve pain and initiate exercises for your home or gym. Lastly we establish a plan with prognosis (expected length and course of recovery). You should bring clothing that is easy to move around freely and possibly sweat in.

Elevate Rehab Methods
Resolve Pain
STEP 1
Resolve any painful tissue using hands-on and exercise interventions that work for you. Starting with an in-depth assessment allows us to identify both what is hurting and the root cause of the issue.
Restore Function
STEP 2
Address impairments that led to the specific issue. This may include restoring range of motion, strength, muscle control, and/or endurance.
Raise Performance
STEP 3
Build strength, power, and speed on the newly regained movements. This is the stage where clients rise to the next level of performance in their sport or activities.
Remain Healthy
STEP 4
Build a sustainable plan to keep healthy and continue progressing long after therapy ends.
